What Is This Form For?

This form is for FOCAS drive-through clinics offering free microchipping. Most, but not all, of our rabies vax clinics include free optional microchipping. Please check the information for the specific clinic you are attending to see whether you need this form. Currently, no online form is available for rabies vaccines. Only use this form if you plan to get your pet(s) microchipped.
